Formstack builds forms, generates documents and collects signatures, and its maker also sells a workflow automation platform under the Intellistack name. Its self serve pricing has two plans, each billed by how many people build forms rather than by how many respond. Forms is $83 a month billed yearly, or $99 billed monthly, for one builder user, 25 forms and 1,000 submissions stored per form. Suite adds document generation and unlimited signatures for $250 or $299, for three builder users, 100 forms and 5,000 submissions per form. Salesforce Solutions and Enterprise are both priced only after a call.
Every current figure here was read on formstack.com/pricing on 4 October 2026, and every older one on the Internet Archive's copies of the same page.

Suite is the only self serve plan with document generation and signatures. Salesforce Solutions wraps native Salesforce forms, document generation and signatures for teams working inside Salesforce, with HIPAA and PCI compliant options. Enterprise adds a dedicated customer success manager, single sign on, custom service level agreements and API access, with unlimited users and submissions. The page says more builder users, forms or submissions can be added to either paid plan, without printing a price for the addition.
What does each builder user cost?
| Forms | Suite | |
|---|---|---|
| Builder users included | 1 | 3 |
| Price a month, billed yearly | $83 | $250 |
| Price a month, billed monthly | $99 | $299 |
| Cost a month per builder user, billed yearly | $83.00 | $83.33 |
| Cost a month per builder user, billed monthly | $99.00 | $99.67 |
A builder user on Suite costs about the same as one on Forms, whichever billing period is chosen. The difference between the two plans is what each user gets to build with, not what each seat costs.
Is there a free plan or a trial?
Formstack runs a 14 day free trial of the Forms product only, with no credit card needed, and full access to its features during the trial. The page lists no trial for Suite, Salesforce Solutions or Enterprise, and no refund terms for any plan. It accepts Mastercard, Visa, Discover and American Express, offers annual invoices on certain plans, and applies its nonprofit discount to invoices while excluding the annual discount from them.
How has Formstack's price changed?
| Archive copy | Plans and prices a month |
|---|---|
| September 2010 | Free for 3 forms; Starter $14 for 5; Professional $29 for 20; Business $59 for 100; Max $159 for 1,000, each with a 30 day money back guarantee |
| July 2012 | The same five plans and prices, now with a 14 day free trial needing no card |
| July 2014 | Renamed: Individual $39 for 20 forms and 2,000 submissions; Manager $99 for 100 and 10,000; Teams $249 for 1,000 and 100,000; Enterprise $599 for 5,000 and 250,000 |
| June 2016 | Renamed again: Silver $39 for 20 forms, submissions cut to 1,000; Gold $99 unchanged; Platinum $249 unchanged; Enterprise moved to a quote |
| July 2018 | Bronze added at $19 for 5 forms; Silver, Gold and Platinum held their prices while their included users fell, from 5 to 1, 10 to 5, and 25 to 10 |
| July 2020 | Bronze held at $19; Silver rose from $39 to $59; Gold and Platinum held at $99 and $249 |
| May 2022 | The pricing page printed no plans or prices, only links out to a separate page for each product |
| July 2024 | Prices returned under new names: Forms $83 or $99 for 1 builder user, 1,000 submissions stored per form and 25,000 in total; Suite $250 or $299 for 5 builder users, 10,000 submissions per form and 1,000,000 in total, with 250 document merges |
| October 2026 | Forms unchanged. Suite held its price while builder users fell from 5 to 3, submissions per form halved from 10,000 to 5,000, and document merges were renamed document deliveries at the same count of 250 |
The pattern repeats across fourteen years: a plan's price holds for years at a time while what it includes shrinks underneath it. Between the 2016 and 2018 copies the included users fell on every plan at unchanged prices, and by 2020 Silver's price had also risen from $39 to $59. In 2022 the pricing page printed no prices at all, and by 2024 two plans were back under new names. Most recently, Suite's $250 price has not moved since July 2024, but a Suite customer today builds with two fewer users and half the submissions per form than one who signed up that month.
Does Formstack run a feature voting board?
Its pricing page links to roadmap.formstack.com under the label Release Notes, and that address leads to a Productboard portal titled "Intellistack Portal | Product Roadmap," with a Submit idea button and Planned and Released tabs listing items for Formstack Forms, Formstack Documents and related Salesforce tools. Opening one of those items shows no vote count, only a prompt asking how important the idea is, with the choices not important, nice to have, important and critical. Formstack runs a public board where customers submit and rate ideas, under its parent company's branding rather than its own.
What this costs
Lite
$9 /mo
Pro
$39 /mo
Lifetime
$999 once
Fourteen day trial of Pro, card asked for at signup. Nothing here counts the people who vote, so the bill does not move when the board starts working. The long version, with what happens if you stop paying.
VoteFirst is a feature request board: customers post requests, vote and follow a public roadmap, at $9 a month for one project or $39 for unlimited projects and colleagues, with no builder user to pay for. Formstack collects data, generates documents and captures signatures, none of which VoteFirst does. Jotform and Typeform price the closest form builders by submission volume, Fillout prices one with a free plan for 1,000 responses a month, the form builder shortlist compares eleven form builders by the response, and the pricing hub groups every vendor by what it meters.
